Reporting to the Manager, Case Management and consistent with the Mission, Values and policies of St.Amant, the Case Worker is a critical role in ensuring the success of St.Amant business and operations. Position Summary:

St.Amant is excited to offer a new Case Management Service to families with children diagnosed with autism throughout Manitoba and will be hiring Case Workers to work in this new program through several phases of expansion.

The Case Worker is primarily a remote position with most meetings and services offered virtually, and some services provided in-person in the client’s home and/or in the community on an as needed basis. The Case Worker provides case management services for children with Autism Spectrum Disorder. The Case Worker is expected to work as a member of a service delivery team and use sound independent judgement. The Case Worker uses the professional application of social work theory and methods to deliver discipline-based services and intervention strategies to individuals, families, agencies and care providers through the following steps of the case management process:

  • Referral and intake
  • Crisis and/or protection services
  • Assessment (including risk assessment)
  • Case planning, counselling and support services provision, including financial management
  • Service monitoring and evaluation
  • Service termination

The Case Worker will require in-depth knowledge of program policies, directives and applicable legislation. The Case Worker will take an active role in promoting service and developing creative strategies to work with families and systems. The Case Worker promotes a positive, nurturing environment for the family and works toward strengthening support networks, enhancing knowledge, competencies, skills, and family stability using a family-centered approach within an ecological model of practice.

  • The Case Worker provides services in accordance with the Mission and Values of St.Amant, policies and procedures of Clinical Services, external legislation and regulatory bodies and all relevant professional standards of practice.
  • The Case Worker is familiar with and promotes a mindful, family-centered, customer service focus in the provision of services to clients, staff and in other professional relationships.
  • The Case Worker promotes processes that contribute to self-determination and the development of skills and competencies of individuals and care providers. This pertains to all areas of intervention including assessments, treatment, and implementation of service.
  • The Case Worker is responsible for evaluating effectiveness and efficiency of interventions to ensure outcomes that are positive and beneficial to clients
  • The Case Worker is familiar with social policy, issues, trends, best practices in the field, needs, gaps or opportunities which may improve the lives of persons with Autism Spectrum Disorder, or which promote social justice, equity or accessibility.
  • The Case Worker is familiar with and adheres to all policies related to the program, risk management and those of workplace activity, promotes and follows all safety, health and respectful workplace practices.   • Qualifications:

    Educational and Professional:

    • Must possess a Bachelor of Social Work (BSW) Degree from an accredited university
    • Must be eligible for and maintain registration with the Manitoba College of Social Workers including meeting all prescribed continuing education requirements of the profession
    • Minimum of two years’ social work experience in the area of developmental disabilities and/or autism spectrum disorder is preferred
    • French language skills are an asset

    Clinical:

    • Demonstrated knowledge and experience using a family-centered model of service provision
    • Knowledge of the history of services to people with disabilities, current trends, philosophies and practices concerning their care, education and lifestyles
    • Knowledge and experience working with children and families and experience working in the area of developmental disabilities/Autism Spectrum Disorder
    • Understand own clinical limitations and seek support and consultation for clinical activities so as to do “no harm”

    Personal:

    • Ability to work with minimal supervision as well as a collaborative team player in a team environment
    • Ability to organize and prioritize work in a fast-paced environment
    • Must possess a valid Manitoba driver’s license and have access to a vehicle
    • Ability to work remotely but also attend in-person meetings in the region assigned
